Reporting Accidents/Injuries/Claims

Reporting Employee Injuries

Report all employee injuries by completing a Notification of Injury Form [doc] available from your school/department secretary. Fax the completed form to the Workers' Compensation Office at (720) 424-8231.

If an employee needs medical treatment, get an Authorization for Treatment Form [pdf] from the school office, department or the Workers' Compensation Office. Seek medical attention at any one of the five DPS Occupational Medicine Clinics. If an injury is life or limb-threatening or for after hours emergency care, go to Rose Medical Center Emergency Room.

Reporting Student Accidents

Report all student accidents by completing a Pupil Accident Report Form [pdf] and sending it to Risk Management via school mail or fax. The forms are available in the school office or may be ordered from the Warehouse in packages of 25 (Stock No. 01-164012).

Reporting Property Losses - Break-Ins, Vandalism, Property Damage, Theft

Notify:

  • Safety & Security: Call (720) 423-3911 to report all break-ins, vandalism, and damaged or stolen property.
  • Police Department - Report all break-ins, thefts and vandalism, (720) 913-2000 for offenses that require a police officer to respond. To obtain a Case Report Number (at least 72 hours after a report is filed), call (720) 913-6029.
  • Facility Management - During normal working hours, call (720) 423-4020 to report facility property damage directly to Facility Management. After hours, call Safety and Security Dispatch at (720) 423-3911.
  • Fixed Assets Department - For all DPS fixed assets (barcoded items), complete an Equipment Activity Report (EAR) and send it via school mail or fax (720) 423-4109 to the Fixed Assets Department/Service Building. Keep a copy for your records.
  • Risk Management - Complete a Property Loss or Damage Report form, include the Case Report Number (see Police Department above), and send via school mail or e-mail yolanda_encinias@dpsk12.org in Risk Management. NOTE: If property is on loan, the borrowing school/department is responsible for the reporting procedure. For questions, call Risk Management at (720) 424-8228.

Reporting Visitor Injuries

Report all visitor injuries using the Pupil Accident Report Form [pdf]. Cross off the word "Pupil" at the top of the form and write in "Visitor". Complete the form and send it to Risk Management via school mail or fax. The forms may be downloaded here or are available in the school office. The forms may be ordered from the Warehouse in packages of 25 (Stock No. 01-164012).

Reporting School/Site Incidents (Safety and Security)

The following incidents shall be reported in writing on the School/Site Incident Report form and submitted by Fax or email to DPS Safety and Security within 24 hours of the incident:

  • Incidents which result in injury to students and/or staff
  • All weapon related incidents
  • All assaults by students or others on another student or staff member
  • All incidents where students are arrested on school property
  • Malicious damages to District property (fire, graffiti, etc.)
  • Losses of district property
  • All sexually related incidents
  • Any incident that an administrator wants documented for future reference
